Membership and Service Fees

Getting started with your membership is easy! Simply call 910-725-6018 or email [email protected] to enroll. Once enrolled we set up your appointment for a phone, video or in-person initial consultation.

Membership Option Fees

Wellness – $10/month

Coordination – $50/month ($95/month/couple)

Continuity – $100/month ($175/month/couple)

A yearly statement is provided, and you will be billed on the 30th of each month covering the upcoming month fee. There is no contract. You may change your membership option at any time. Cancelled memberships are effective the 1st day of the following month.

Aging Life Care Fees (Optional/As Needed service paid separately from Membership)

With Wellness Membership – $67.50/hour (Billed separately from Membership Fee)

With Coordination Membership – $45.00/hour (Billed separately from Membership Fee)

With Continuity Membership – $22.50/hour (Billed separately from Membership Fee)

Without Coordination or Continuity Membership – $90 hour

Payment Options

Automatic recurring monthly payment is available with a Debit/Credit Card. (To stop or make changes to an established recurring payment please call 910-725-6018.) {LINK}

Quarterly pre-payment can be done with check, or debit/credit card. Send payment to 334 Mill Creek Rd, Suite D, Carthage, NC 28327 or call 910-725-6018 to pay by card.

You may also use a debit/credit card to make a non-recurring payment. {LINK}

Insurance

Membership fees and Aging Life Care fees are not covered by insurance. Some of St Luke’s Center for Senior Health and Wellness membership benefits are covered by insurance such as Chronic Care Management, Physician Medical Visits (St. Luke’s At Home), Health Education and Remote Physiologic Monitoring. We accept and participate in all major Medicare, Secondary and North Carolina insurance.

HIPAA and Confidentiality

St. Luke’s Center for Senior Health and Wellness complies with all healthcare HIPAA and confidentiality regulations. All medical information/communication is stored on HIPAA compliant electronic medical record, billing and communication platforms. A copy of our privacy policy is available for you to read at request.
